* IN ORDER TO PARTICIPATE IN TRY-OUTS/PRACTICE, ALL ATHLETES MUST HAVE A VALID BASELINE CONCUSSION       

   TEST ON FILE (via concussionvitalsigns.com) 

* Testing is required every 2 years.  Typically, athletes test their Freshman and Junior years.  

   However, if you DID NOT take the test last school year (for a Fall, Winter or Spring sport), then you need to take it.

* NO TEST = NO TRY-OUTS/PRACTICE

* READ ALL DIRECTIONS CAREFULLY to avoid getting an invalid test, and having to retake it.

* When signing in to take the test, INPUT YOUR STUDENT ID# (NUMBERS ONLY!) & YOUR FIRST AND LAST NAME 

* Out of district athletes - use Date of Birth in place of Student ID
