 The following forms must be completed online in the Genesis Parent Portal, accessed in the “Forms” tab: 

1. Activity / Athletic Eligibility Form: Must be completed EACH SEASON by the submission deadline for students participating in multiple sport's seasons.
2. Health History Update Form: Must be completed EACH SEASON (after the initial season covered by the original physical form) by the submission deadline for students participating in multiple sport's seasons. Please explain all “yes” answers thoroughly.
3. Athletic Emergency Information: Must be completed ONCE each academic year.
4. Sports-Related Concussion and Head Injury Fact Sheet: Must be completed ONCE each academic year.
5. The Sudden Cardiac Death in Young Athletes Information Sheet: Must be completed ONCE each academic year.
6. NJSIAA Steroid Testing Policy: Must be completed ONCE each academic year.
7. Use and Misuse of Opioid Drugs: Must be completed ONCE each academic year AND Athletes vs. Opioids video must be viewed each season.
8. Sports-Related Eye Injuries Fact Sheet: Must be completed ONCE each academic year.
9. Spectator Code of Conduct Policy: Must be completed ONCE each academic year.
10. Parental Transportation Form‐ Must be completed ONCE each academic year.
